Referral Process

Referrals for Pendarren Court are coordinated by the Registered Hospital Manager, based at Pendarren Court. Referrals are welcomed from health care professionals such as doctors, social workers or case managers. All referrals received will be confirmed with the patient’s Responsible Clinician (RC) and funding health authority prior to processing.

A referral pack will be completed for all proposed admissions to Pendarren Court.

All referrals will be discussed by the multi-disciplinary Admissions Panel, which meets on a weekly basis, and is chaired by the Clinical Director.

If the admission criteria for Pendarren Court are met, appropriate clinicians will be identified to undertake the assessment and may include a psychiatrist, psychologist, occupational therapist or nurse, but will always consist of those with the skills to carry out a comprehensive and detailed assessment. The assessment process will include a mental state examination, history of illness, premorbid history, social and education history, current issues (including risk of harm to self and others) and will lead to the formulation of the current and ongoing needs of the patient and how the team can meet the patient’s immediate and future needs.

An initial referral assessment report will be produced by the referral team, who will again discuss the needs of the patient and the ability of the service to provide appropriate care and a decision taken about suitability for admission.

This decision will be communicated to the initial referrer.
